Mission: The Armed Forces Wellness Center (AFWC) provides integrated and standardized programs and services that promote enhanced and sustained healthy lifestyles to improve the overall readiness of the Army.

Vision: Lead in the delivery of the most effective primary preventative health and wellness services that improve the overall well-being of Service Members, Family Members, Retirees, and Department of Defense Civilians.

The AFWC approach to service is holistic. Our staff members take into account all of an individual's physical, psychological and social circumstances when providing services. A holistic approach is important because a person's health cannot be fully addressed unless we consider the whole person.  Our Health Educators deliver primary prevention programs that will help you create and meet your health and wellness goals. You will set your goals; and AFWC staff will help put you on the path to achieving them while walking the path with you!

Health Assessment Review (HWT) - A questionnaire that’s used to determine the current level of an individual’s health and identify the changes that will be necessary to reduce risk factors for disease prevention, and ability to increase physical activity safely.

***Prior to your AFWC appointment, you are required to fill out the Health and Wellness Questionnaire (HWQ).

Instructions:

  • Please log in or create an account at https://awc.army.mil
  • Utilize your own social security number if you do not have a CAC.
  • Complete all 6 sections of the Health Assessment at least 24 hours prior to your appointment.
  • HWQs are updated every 30 days.  If it has been greater than 30 days since your last submission, please complete again. If you are unable to complete the Health Assessment, please arrive 15 minutes early for your appointment to complete.

Physical Fitness - The Physical Fitness Assessment focuses on health related physical fitness components such as cardio-respiratory fitness (VO2 Max & Submax), body composition (BodPod), muscular fitness (strength and endurance), and flexibility, which have strong relationships with good health, characterized by an ability to perform daily activities with energy, and are associated with lower prevalence of chronic disease and health conditions and their risk factors. 

Results are used to create individualized exercise prescription to help you achieve your goals. 

  • Appointment Preparation Cardiorespiratory Fitness (VO2) Testing
    • No food, exercise, drink, or nicotine 5 hours prior to appointment
    • No moderate exercise 5 hours prior
    • No vigorous exercise 14 hours prior
  • Appointment Preparation for Body Composition Testing (BodPod)
    • No food, exercise, drink, or nicotine 2 hours prior to appointment
    • Male Attire: Spandex/Lycra shorts, boxer briefs, or Speedo, NO Cotton
    • Female Attire: Spandex/Lycra Sports Bra no padding or wire with Spandex/Lycra shorts/capris, or bathing suit no padding or wire, NO Cotton

Healthy Nutrition - Use of metabolic testing that synchronizes an individual's resting metabolic rate to provide tailored strategies for weight loss, gain, or maintenance.

Stress Management - Provide education and training on biofeedback, stress relief techniques, and positive coping skills. Deep breathing, guided imagery, progressive muscle relaxation, and mindfulness meditation are utilized. 

General Wellness Education - Classes upon request on healthy lifestyles, nutrition, sleep, stress, metabolism, and performance optimization.
