Williamsburg Community Based Medical Home

The Williamsburg Medical Home affords TRICARE Prime beneficiaries the opportunity to enroll and receive patient centered care within the greater Williamsburg community.  A Patient Centered Medical Home is a healthcare delivery model designed around one core principal – the patient. The model provides comprehensive primary care, promotes wellness through empowering patients, and seamlessly coordinates care within the Tidewater Military Health System to achieve optimal health.  Services include: Adult and Pediatric Primary Care, Physical Therapy, Laboratory, and Pharmacy.

Services Offered

Immunizations:  Immunizations for infants, children and adults are available for well baby care, school, employment and travel.

Women’s Health:  Routine gynecologic examinations, pap smears, mammogram referrals and contraceptive counseling are available.

School and Sports:  Physicals to enter primary and secondary Virginia schools, any college and for sports are available by appointment only. Please bring the appropriate school forms to your appointment.

Prevention:  Your care team will help you be proactive in staying healthy with the latest screening and  prevention recommendations.

Medical Management:  Manage chronic conditions such as asthma, diabetes, and hypertension.

Nurse Case Management:  Special criteria must be met for this service and your PCM will initiate these services.

Mondays thru Fridays from 9-11 a.m. and 1 - 4 p.m. ; No PPDs (TB Test) will be placed on Thursdays.

How to Fill a Prescription
New Prescriptions
If you want to fill a new prescription at Williamsburg CBMH, follow these steps:
Paper Prescription

  • Bring paper prescription from provider to the pharmacy – will result in a 2 to 3 business day turn around for processing

e-Prescription

  • Providers search for “DoD Fort Eustis” when sending electronic prescriptions

Call the Pharmacy at 757-914-6353 if you have questions on filling your new prescription.
Refills
If you want to refill a prescription at Williamsburg CBMH, follow these steps:

  • Call 757-953-2500

If you’re out of refills, contact your doctor for a new prescription before coming to the pharmacy.
